本人自愿参加OCPA举办的各类活动（包括但不限于聚餐、晚会、讲座、烧烤等），了解参与可能存在轻微受伤、食源过敏引发的疾病或财产损失等一般风险，并自行负责人身安全与财物保管。By participating in any OCPA event (including but not limited to potlucks, galas, seminars, BBQs, or outdoor gatherings), I acknowledge that participation is voluntary and may involve ordinary risks such as minor injuries, foodborne illness, food allergy, or property damage. I am responsible for my own safety, health, and belongings.

本人同时对随行家人（包括儿童及长者）的安全与行为负全部责任，并同意他们的参与为本人自愿决定。I also understand that my accompanying family members, including children and seniors, are under my care and supervision at all times. I accept full responsibility for their safety, conduct, and participation in all event activities.

在法律允许范围内，本人同意免除并不追究OCPA、其理事、志愿者及场地方的相关责任（除非存在重大过失或故意行为）。To the fullest extent permitted by law, I release and hold harmless the Orlando Chinese Professionals Association (OCPA), its officers, volunteers, and venue partners from any liability, claim, or damage arising from participation, except in cases of gross negligence or willful misconduct.

本人亦同意OCPA可使用活动中拍摄的照片或视频用于非商业宣传，如不愿被使用，可提前书面通知主办方。I also grant OCPA permission to use event photos or videos containing my likeness for non-commercial promotional purposes, unless I request otherwise in writing.
OCPA 会员条款
OCPA Membership Terms

会员有效期。 会员资格自以下两者较晚日期起生效：① 提交申请之日；② 会费到账并确认之日。会员有效期至下一次 OCPA 年会及中秋庆典结束（下称“会员年度”）。Term of Membership. Membership becomes active on the later of (a) the date your application is submitted and (b) the date your membership dues are received and confirmed. The membership remains valid through the conclusion of the next OCPA Annual Conference and Chinese Mid-Autumn Festival Celebration (the “Membership Year”).

不予退款／不可转让。 会费不予退款，会员资格不可转让。Non-Refundable / Non-Transferable. Membership dues are non-refundable and memberships are not transferable.

续费。 会员资格在会员年度结束时自动到期，如需继续享受权益，须在下一周期续费。Renewal. Membership expires at the end of the Membership Year and must be renewed to continue benefits in the next cycle.

会员权益。 在会员年度内，会员可免费参加由 OCPA 举办的活动，包括但不限于中秋晚会（月饼免费，数量有限）、春游/烧烤或夏季海滩活动、讲座/研讨会及 OCPA 另行公布的活动。点击查看会员福利详情。 Member Benefits. During the Membership Year, members receive complimentary admission to OCPA-hosted activities, including (but not limited to) the Mid-Autumn Festival party (free mooncakes while supplies last), spring outing, BBQ or summer beach party, seminars, and other programs announced by OCPA. 活动说明。 部分活动可能需预约、名额有限，或因故调整/取消。除非另有说明，会员权益不包含个人支出（如交通、停车、第三方费用、纪念品等）。参加活动需同意OCPA活动免责条款，条款细节如上。Event Conditions. Some activities may require RSVP, have capacity limits, or be rescheduled/cancelled. Benefits do not cover personal expenses (e.g., travel, parking, third-party fees, merchandise) unless expressly stated. When participate an OCPA event, you must agree to the OCPA Event Liability Waiver as above.

政策与行为规范。 参加活动须遵守 OCPA 各项政策、场地规定及文明礼仪要求。Policies & Conduct. Participation is subject to OCPA policies, venue rules, and a respectful code of conduct.

By submitting this form, you consent to OCPA collecting and using your contact information—such as name, address, email, WeChat ID and phone number—and membership details (e.g., membership type, earliest year, interests) solely for membership administration, event communications, and required record-keeping.
